The Associated Press reported that the State Department security chief and two others have resigned following a scathing report about the Sept. 11 Benghazi attack released Tuesday night.

The report detailed “systemic failures and leadership and management deficiencies at senior levels.”

The Sept. 11 attack resulted in the deaths of Amb. Chris Stevens and three other Americans.
